Please fill out this form completely and transmit it to us prior to any discussion of contracting for the services of one of our stud dogs. </div> <div> 1. Health clearances:</div> <div> OFA grade and number (please send a photocopy of the form).</div> <div> CERF number (please send a photocopy of the form).</div> <div> Pre-breeding workup, including CBC, aerobic and mycoplasma culture/sensitivity (through appropriate guarded culture instrument, (obtained from the area of the cervix), brucella and T4. Original lab report must be forwarded to us. </div> <div> Other:</div> <div> 2. Your detailed plans for the placement of puppies:</div> <div> How many do you intend to keep?</div> <div> How many have you taken reservations for?</div> <div> How do you plan to find homes for the remainder?</div> <div> Send a copy of the contract and questionnaire you use for prospective puppy buyers.</div> <div> 3. Describe your arrangements for a whelping area.</div> <div> What are your fencing/kenneling arrangements?</div> <div> What arrangements do you have to house and care for those puppies for which you cannot find homes?</div> <div> 4. Briefly describe your prior whelping experience; how many litters in total have you whelped?</div> <div> How many litters has this bitch had previously?</div> <div> How many breeding attempts have been made with this bitch?</div> <div> 5. What show, trial, training, etc. experience has this bitch had?</div> <div> What is her record at these events?</div> <div> What titles has she earned?</div> <div> Has she stayed away from home before?</div> <div> 6. This bitch needs to be evaluated for breeding quality. Photos or a video should be sent right away, and a personal examination must be set up to determine this. State below what arrangements for such an evaluation you are prepared to make.</div> <div> 7. What form of positive identification does the bitch carry? (Microchip or tattoo)</div> <div> What is the number?</div> <div> If a chip, which kind?</div> <div> What is the location of this identification?</div> <div> 8. Enclose a copy of the bitch's AKC registration, pedigree, and if an import, the export pedigree of the country of origin.</div> <div> 9. Enclose a copy of the bitch's veterinary records, including current vaccinations and dates, parasite examination results and date, medications currently being administered, medications administered within the past year, and any existing medical conditions. State any congenital medical problems which have been diagnosed.</div> <div> When these materials have arrived, you can call to talk to me regarding the stud dog you prefer to use, his stud fee, travel arrangements or semen shipping arrangements, and any other matters which seem unclear.</div> <bR> <DIV ALIGN="LEFT"></DIV> <div> STUD SERVICE CONTRACT </div> <bR> <div> Thank you for expressing an interest in using one of our dogs in your breeding program. Before a commitment for breeding is made please consider the following. Prior to a breeding agreement, we require from you:</div> <div> 1. A photocopy of your bitch's AKC / CKC registration. </div> <div> 2. A legible four generation pedigree. </div> <div> 3. A photocopy of your bitch's OFA hip and elbow certification.</div> <div> 4.  Copies of tests below. </div> <div> 5. Recent photograph or video tape of the bitch.</div> <div> Following an agreement to breed, it is required that you:</div> <div> 1. Provide a veterinary certificate of a negative Brucella titer, negative aerobic vaginal culture taken with guarded culture instrument, and a negative mycoplasma culture, dated no more than 30 days before the mating. We will need a photocopy of all lab reports. </div> <div> 2. Pay all transportation costs for the bitch. Airport pick-up and delivery fees to DFW Airport as follows: $10.00 </div> <div> (To be paid before the bitch is shipped.) </div> <div> 3. Pay all veterinary costs, at the time of service, while the bitch is in our care. This would include vaginal smears, artificial insemination (if the bitch is unreceptive to natural breeding or we are unhappy with the culture report etc. ) </div> <div> 4. Pay all expenses if using fresh chilled semen including collection kits, semen collection, shipping charges, etc. </div> <div> 5. A record of current vaccinations including Rabies. </div> <div> 6. Names and addresses of all prospective puppy owners. </div> <div> 7. Agree that puppies resulting from this breeding shall not knowingly be sold either directly, or to an agent thereof, who sells outright or consigns to a pet shop, kennel, or individual whose purpose is mass production, wholesaling, or retailing of purebred dogs. </div> <div> Prior to an agreement to breed, our responsibilities to you are as follows:</div> <div> 1. To provide a photocopy of our stud's AKC registration. </div> <div> 2. To provide a photocopy of our stud's OFA hip and elbow certification. </div> <div> 3. A four generation pedigree. </div> <div> After an agreement to breed has been signed, it would be our responsibility to provide you: </div> <div> 1. A veterinarian certificate of a negative Brucella titer dated no more than 30 days before the mating. </div> <div> 2. A veterinarian certified semen evaluation dated no more than 90 days before the mating. </div> <div> All proposed bitches must meet the following criteria: </div> <div> 1. The bitch must be in good physical condition. She must be free of fleas and ticks and have had a bath before she arrives. </div> <div> 2. The bitch must have a stable temperament. </div> <div> 3. The bitch must have proper dentition. We will not breed any bitch that is under or overshot or has four or more missing teeth. </div> <bR> <div> STUD SERVICE POLICY </div> <bR> <div> 1. The stud fee for ____ is $_____. Half is payable upon the bitches arrival. The balance is due when the puppies are one week of age. </div> <div> 2. Two or more live puppies, seven days old, constitutes a litter. </div> <div> 3. If a breeding does not occur, either naturally or artificially, the owner of the stud dog will return the stud fee. In this case, a five dollar per day board charge charge will apply. </div> <div> 4. If the bitch does not whelp two or more live puppies, a return stud service will be granted. </div> <div> 5.
